This briefing forms part of the Making it Happen resources and focuses on getting the transition process right in order for young people to be able to focus on building a healthy fulfilling adult life in the same way as tehir peers.

The Transition from childhood through adolescence into adulthood is an important and formative experence for everyone. Experiences in this period have a huge influence on the rest of a person’s life, and this is no different for those with SEN and disability. This group do, however, have to deal with some additional complexities when moving from one set of support systems in children’s services to what can seem like a completely different adult system.
